We will miss you, but before you go we need the following form so that we will stop billing you

• UCAR Membership Form --make sure to have it signed by your broker

Terminate ReaderKEY

You may return your key to UCAR for a deposit refund. Keys must be received within 30 days of terminating your membership with UCAR, in good condition, up to date on the lease, and include the USB cable that accompanied it. If the cable is not included, $10 will be deducted from your total deposit amount. Your key will not work at another board.

Terminate Risco Lock Boxes

If you would like to sell your boxes, you may transfer your boxes to another board with you, sell them to another agent or sell them to UCAR for $60.
